Privacy.

This notice explains what information the Anuvi Medical & Walk-in Clinic website collects, how we use it, and the choices available to you. It applies to this website and its online forms. Health information collected when you receive medical care is handled separately under Alberta’s Health Information Act.

The short version

You can browse most of this website without giving us your name or contact information. If you submit a form, we collect the information you enter so that we can respond to you or manage the new-patient list.

The website and the services that support it may also process limited technical information such as your IP address, browser information, and website traffic data. We do not sell your personal information or use it for third-party advertising.

Information you provide

The new-patient form asks for your name and phone number. You may also provide your email address and tell us whether you are looking for care for yourself, your family, or a dependent.

The contact form asks for your name, email address, and message.

Please do not use either form to send medical information, photographs, requests for medical advice, or urgent health concerns. These forms are intended for general communication and new-patient list management. They are not a substitute for speaking with a healthcare professional and are not monitored continuously.

Call 911 in an emergency. Call 811 for health advice if you are unsure where to seek care.

How we use your information

We use information submitted through the new-patient form to manage the new-patient list and contact you about registration or appointment availability.

We use information submitted through the contact form to read and respond to your message and, where appropriate, to follow up on your request.

Submitting a form does not guarantee acceptance into the family practice, create a physician-patient relationship, or guarantee that the clinic will respond within a particular time.

We may also use or disclose information where required or permitted by law, to protect someone’s safety, to investigate misuse of the website, or to obtain professional advice.

Service providers

We use service providers to operate the website and deliver form submissions:

Cloudflare hosts and protects the website. It may process technical information such as IP addresses, browser information, traffic data, and security information.

Web3Forms processes form submissions and forwards them to the clinic’s email inbox. Its servers are located in the United States. Web3Forms states that it does not store form submissions, although limited server logs may be kept temporarily.

Google Maps is used in the website’s location section. When the map loads, Google may receive technical information such as your IP address and information about your device or browser.

These providers process information according to their own privacy practices and may process information outside Canada. Information processed outside Canada may be subject to the laws of the country where it is processed.

How long we keep information

We keep personal information only for as long as reasonably needed to manage the new-patient list, respond to a message, meet our operational or legal obligations and maintain appropriate records.

New-patient list information may be kept until you have been contacted, registered, removed from the list, or the clinic no longer needs the information for that purpose.

Contact messages may be kept while we respond to the enquiry and for a reasonable period afterward. If a message becomes relevant to medical care, legal obligations, or patient safety, it may need to be retained or transferred to an appropriate record.

When information is no longer reasonably required, we will delete it, destroy it securely or remove identifying information, as appropriate.

Cookies and website preferences

This website does not currently use advertising cookies or third-party advertising trackers.

The website stores your selected language on your device so that it can remember whether you prefer English or Spanish. Cloudflare or other service providers may also use limited technical measures where needed for website security, traffic management, or fraud prevention.

Protecting your information

We use reasonable administrative and technical safeguards to protect personal information. Access is limited to people who need the information for their work.

No website, email service, or internet transmission can be guaranteed to be completely secure. Please do not submit sensitive medical information through these forms.

Health information and medical records

Physicians are custodians of health information under Alberta’s Health Information Act. Clinic employees, contractors and others who provide services on behalf of a physician may act as affiliates under that Act.

Health information collected when you receive medical care is handled through the clinic’s clinical systems and privacy practices. You may ask to access or correct health information held by your physician, subject to the Health Information Act.

Your choices and privacy rights

You may contact us to:

• ask what personal information the clinic has about you;

• request access to or correction of your personal information;

• update your contact information;

• withdraw your consent where applicable; or

• ask to be removed from the new-patient list.

Some information may need to be retained where permitted or required by law. Withdrawing consent does not affect uses or disclosures that occurred before consent was withdrawn.

Changes to this notice

We may update this notice when our website, service providers or information-handling practices change. The date shown at the top will identify the latest version.

Contact the clinic and ask for the Privacy Officer if you have a question, want to make a request or believe your information has not been handled appropriately. You may also contact the Office of the Information and Privacy Commissioner of Alberta.
